A flat and supple abdomen is the final aim of hours of physical
training and diets: surgery can help reduce skin laxity, excess skin
and part of the excess fat, but patients must always remember that
surgical procedures will never take the place of a diet and the aim is
not weight loss. Patients with a moderate localized abdominal fat
excess and elastic and supple skin can undergo liposuction on its own.
If skin excess or laxity involve the lower part of the abdomen
(below the navel) the patient can be a candidate for
"mini-abdominoplasty", a surgical procedure in which the skin flap is
not separated beyond the navel.
Recovery time after surgery is shorter compared to the complete procedure.
Abdominoplasty with navel transposition (see diagram) is indicated
in the presence of skin laxity that involves all the abdomen. The skin
flap is separated from the abdominal wall in the pubic region up to the
lower part of the costal arch.
In order to obtain a flatter abdomen abdominoplasy is often associated
to suture of the fascia of the abdominal rectus muscles in the case of
diastasis (separation of muscles) due to pregnancies or changes in
posture. This can be done in association with liposuction on the hips
or the upper part of the abdomen.
If the patient presents with a large volume of abdominal fat and not
much excess skin the best indication is for a two-stage procedure: the
first stage consists in liposuction of the abdominal area and 10 months
later the second stage consists in abdominoplasty to remove the excess
skin. The two-stage technique is performed to avoid the post op
complications that seem to increase when the two procedures (widened
liposuction and abdominoplasty) are performed together.
The length of the scar depends on the amount of skin and fat that are removed.
The scar is often a brown/red colour and slightly raised for 12 months at the most.
Contraindications for abdominoplasty are: diabetes, smoking,
previous episodes of venous thrombosis, overweight, pregnancy
programmed for the near future, weight loss for diet in that period (it
is better to wait until the end of the diet).
Complications: infection, haematoma, deep venous thrombosis (rare), change in skin sensitivity
How long does surgery take?: about 3 hours
Anaesthesia: general
Hospitalization: 24-72 hour in hospital (depending on abdominal volume)
Post-op: antibiotics for 7 days, drains will be removed after 24-48 hours, compressive abdominal girdle for 8-12 weeks.
Time off work 2 weeks. No sport activities for 6/8 weeks.
